How many questions are on the NREMT?
Short answer: it depends on how you're doing. The NREMT cognitive exam is computer-adaptive — the number of items you see is decided by the algorithm, not the clock.

Why the range?
The NREMT uses computer-adaptive testing (CAT). The system starts you with a medium-difficulty item, then re-estimates your ability after every answer. It keeps serving items until your ability estimate is 95% above or below the passing standard — or until you hit the maximum item count.
That's why two candidates can walk out with wildly different question counts and both pass. Your score is a difficulty-weighted ability estimate, not a percent correct.
What "pilot items" are
Every exam includes about 10 unscored pilot questions that NREMT is testing for future exams. You will never know which ones. Answer every question as if it counts.

Passing standard
NREMT does not publish a numeric percentage to pass. The passing standard is set by a criterion-referenced study (Angoff method) and represents the minimum competency for entry-level practice. Your score report is pass/fail, plus a "near passing" / "below passing" / "above passing" band for each content area on a fail.

- How many questions are on the NREMT EMT exam?
- Between 70 and 120 items on a computer-adaptive test. The exam ends when the algorithm is 95% confident you are above or below the passing standard.
- Does the NREMT get harder or easier as you go?
- Both. It's adaptive: correct answers make the next question harder, incorrect answers make it easier. The final score is not percent correct — it's a difficulty-weighted ability estimate.
- Why did my exam stop at only 70 questions?
- Either the algorithm decided you were clearly passing, or clearly failing, within the minimum item count. A short test is not automatically a pass or a fail.
- How much time do I get per question?
- Roughly a minute on EMT and AEMT, a minute on Paramedic. Most candidates finish with time left. Don't rush — reading the stem twice is faster than picking the wrong answer.
- Are there breaks?
- You can leave for the bathroom, but the clock keeps running. There are no scheduled breaks.
