EMS Pharmacology
Pharmacology questions are the easiest points on the NREMT if you know the scope drugs cold. They're also the easiest to lose if you memorize doses without contraindications.

EMT-scope medications
The exam expects you to know when NOT to give each of these more than when to give them.
- Oxygen, aspirin, nitroglycerin, epinephrine auto-injector
- Oral glucose, albuterol MDI/neb, naloxone (IN/IM)
AEMT additions
IV/IO fluids, D10, glucagon, and often nitrous oxide and additional airway meds. Scope varies by state — know yours.
Paramedic pharmacology
ACLS drugs (epi, amiodarone, adenosine, atropine), analgesics and sedatives, RSI meds in many systems, plus vasopressors like norepinephrine or dopamine.
The high-yield interactions
Nitroglycerin + PDE-5 inhibitors (sildenafil, tadalafil) = severe hypotension. Aspirin in true allergy or active GI bleed. Epinephrine caution in elderly cardiac patients — but never withhold it in true anaphylaxis.

- How long after Viagra is nitroglycerin safe?
- At least 24 hours after sildenafil/vardenafil, and 48 hours after tadalafil. When in doubt, hold the nitro.
- Do I give aspirin for stroke?
- No. Aspirin is for suspected ACS. Prehospital aspirin is not indicated for undifferentiated stroke.
