Worked example
Order: fentanyl 1 mcg/kg for a 176 lb patient. 176 ÷ 2.2 = 80 kg. Dose = 80 × 1 = 80 mcg. If concentration is 50 mcg/mL, draw 80 ÷ 50 = 1.6 mL. (Convert mcg to mg where applicable — 1 mg = 1,000 mcg.)

What formula does this use?
Dose to give (mg) = weight (kg) × dose (mg/kg). Volume to draw (mL) = dose to give (mg) ÷ concentration (mg/mL).
How do I convert pounds to kilograms?
Divide pounds by 2.2. Toggle the lb switch on the calculator to do it automatically.
What if the concentration is written as a ratio?
Convert to mg/mL first. Epinephrine 1:10,000 = 0.1 mg/mL. Epinephrine 1:1,000 = 1 mg/mL.
How do I handle pediatric max doses?
